From Compliance to Confidence: What Leaders Actually Need from Assurance

Thought for the Week:

Assurance is often treated as a compliance activity.
Reports are produced, reviews completed, and evidence compiled.

But senior leaders rarely ask for more assurance paperwork.

What they want — often implicitly — is confidence in the decisions being made .

Confidence that risks are genuinely understood.
That the trade-offs behind decisions are explicit.
That reassurance has not quietly replaced judgement.

Yet assurance reporting often delivers something else:

Volume instead of clarity.
Status instead of insight.
Comfort instead of challenge.

When that happens, assurance risks weakening the very trust it is meant to support.

Effective assurance advice does three things well.

First, it separates fact from judgement.
Decision-makers need to know what is known, what is assumed, and where professional judgement begins.

Second, it explains why a position is reasonable — not just defensible.
A defensible argument may survive scrutiny later.
A reasonable one helps leaders make better decisions now.

Third, it makes uncertainty visible.
Complex infrastructure projects rarely operate in stable conditions.
Assurance that hides uncertainty may feel reassuring, but it rarely improves decision quality.

This is particularly true in ALARP reasoning.

ALARP decisions are not strengthened by more analysis.
They are strengthened by clear reasoning and transparent assumptions .

The most valuable assurance conversations therefore surface trade-offs, test leadership intent, and clarify accountability.

Assurance does not remove responsibility from leaders.
It helps them carry it well.

A simple question often reveals whether assurance is working:

If assurance advice disappeared tomorrow, would leadership still feel confident in its risk decisions?

The answer usually tells you everything.

Effective Compliance Statements: Best Practices for Assurance

An engineering compliance statement is a critical element of many assurance documents, including Hazard Records and Hazard Logs. It demonstrates that a system or project team is complying with applicable standards and regulatory requirements.

However, too often, a compliance statement is reduced to a simple document reference number, offering little justification or detail. This approach is not recommended, as it frequently results in additional questions, rework, and avoidable costs later in the assurance process.

A well-constructed compliance statement should clearly explain the actions taken by the system or project team to achieve compliance. It must be robust, traceable, and comprehensive, providing assessors with the information they need to understand how and why requirements have been met.

It’s also important to remember that the audience often includes independent assessment bodies. The goal is to enable efficient review, not to burden assessors with excessive documentation. Clear, focused compliance statements help reviewers quickly identify what matters most.

When Things have gone BAD

Bringing in specialist help when things have already gone wrong is often a difficult — and costly — process. However, specialist engineering support, such as that provided by system safety engineers, doesn’t have to be approached this way, nor does it need to be full-time.

High Speed Rail Solution announces the appointment of Kyle Booth as Vice President of Operations (North America)

High Speed Rail Solutions (HSRS) is pleased to announce the appointment of Kyle Booth as Vice President of Operations (North America) , marking a key milestone in the company’s continued expansion across the region.

Booth brings extensive experience in systems integration, operational technology, and engineering leadership within complex transportation and infrastructure environments. He previously served as Director of Operational Technology & Systems Integration and Systems Integration Authority at Crosslinx Transit Solutions – Constructors, where he played a pivotal leadership role on the Eglinton Crosstown LRT project in Toronto.

During his tenure at Crosslinx, Booth led and mentored a large multidisciplinary team responsible for integrating more than 40 complex subsystems spanning communications, safety, control, and data platforms. He oversaw the full systems integration lifecycle, including engineering reviews, testing and commissioning, operations readiness, verification and validation, systems assurance, and RAM/safety processes. Booth also represented the project in senior technical and contractual discussions with Metrolinx, Infrastructure Ontario, the Toronto Transit Commission, and major delivery partners, contributing significantly to one of Canada’s most complex transit integration programs.

Following his work at Crosslinx, Booth held senior leadership roles with Ultegra Systems and Tandem Systems Group, where he further advanced solutions in operational technology, cybersecurity, applied AI, and systems integration across sectors such as transit, water and wastewater, airports, smart buildings, and other mission-critical environments.

In his new role at HSRS, Booth will oversee North American operations, strengthen relationships with clients and industry partners, support the growth and development of the Canadian team, and enhance the company’s safety engineering and assurance capabilities.

To make your ICDs effective, we recommend including the following key information:
• Purpose of Interface Control: Clearly outline the goals.
• Project Introduction: Provide context, including all interfaces.
• Interface Overview: Briefly describe each interface, its purpose and the parties involved in the successful integration.
• Assumptions, Constraints, Risks: Identify specific interface-related assumptions and the risks that could materialise.
• Detailed Interface Requirements: Cover functional and physical requirements (e.g. space proofing, data transfer etc).
• Verification and Validation Methods: Specify lifecycle activities to prove compatibility and who is responsible for providing compliance evidence at each stage.

A Picture Paints a Thousand Words

When it comes to understanding engineering changes to railway infrastructure, clarity is key. Yet, many System Definition (SD) documents consist of pages of dense text, making it challenging for readers to fully grasp the changes being made.

A System Definition is essential for complying with CSM RA Regulations, but it’s also crucial that it clearly communicates the scope of the engineering change. Instead of relying solely on long paragraphs, why not use diagrams to make the changes more understandable? Here are a few visual tools that can significantly improve comprehension:

• �� (��): Define the system boundaries and interactions with its environment—one of the most effective diagrams to include in an SD.

• �� : Clearly define the geographic and functional boundaries of the changes.

Missing Evidence

So… it’s nearly time to submit your safety justifications but you don’t have several key pieces of evidence. Don’t panic!

Given the review periods required by external and internal independent assessment/approval bodies (e.g. AsBo, SRP) it is inevitable that there will be pieces of compliance evidence outstanding at submission time.

Be clear in your safety justification as to what evidence is missing or incomplete and be clear on what is required, why it is required, when it is required by, who will provide this evidence and what the consequences would be if it were not provided in time.

Be sure to also raise this in discussion with the independent bodies before and after you have made your submission. Be clear on how the outstanding evidence can be ‘drip-fed’ to assessors to demonstrate that the required information is being provided as required so safety requirements are being mitigated and closed.

Is your Hazard Record out of Control?

The Hazard Record, sometimes referred to as the Hazard Log, is arguably the most important document in demonstrating compliance with the CSM-RA Regulations, and in your project being safely integrated into the existing railway network.

As well as being one of the most important documents, it is also the most troublesome to get right, and can lead to unsupportive Safety Assessment Reports from the independent Assessment Body (AsBo). This will throw the chance of endorsement from System Review Panels into serious doubt.

The outputs from the hazard identification and analysis stages need to be captured and managed in the project Hazard Record. The Hazard Record is a formal record of the system safety hazards and their causes identified, based within the scope of the System Definition.

Some of the common problems we find with Hazard Records are as follows:

Formatting can be quite sporadic,
Revision history is often severely lacking in detail,
Traceability, especially with regards to where and when each hazard was raised, is not always present,
The Hazard Record can be cluttered with non-CSM related hazards,
Poorly worded entries with descriptions that are easy to misinterpret.

This can all come about due to projects NOT:

Understanding the importance of the Hazard Record,
Assigning dedicated personnel to the role of Hazard Record Manager,
Holding formal reviews of Hazard Record entries,
Including appropriate evidence through each stage of the project lifecycle.

So, now that you have a better understanding of the importance of the Hazard Record, you may find it worth having a review of your project’s Hazard Record to make sure you don’t end up in an awkward and time consuming situation when it matters most.

Walking for Hope: HSRS Joins the Fight Against Suicide with PAPYRUS

On October 10, 2024, Peter Williams and Michael Hinton proudly joined the HOPEWALK in Swindon for World Mental Health Day, raising awareness for suicide prevention.

Michael initiated conversations with PAPYRUS , our office neighbors, after seeing the HOPEWALK advertised, and it was great to see these efforts come to life. Last week, Alex from PAPYRUS presented at our Quarterly Brief, sharing their story, offering training, and discussing volunteer opportunities.

Peter reflected on his personal experience, saying, "Having recently experienced the loss of someone very dear to me to suicide, I was keen to do something that could help others. The HOPEWALK with PAPYRUS has given me that first opportunity, joining other members of our community and spreading awareness of a great organization that provides incredible support. Moving forward, I am very keen to complete the training courses available to better equip myself to support people who are struggling or in need of empathy, compassion, or just someone to talk to."

Michael added, "Breaking the stigma and getting people talking about how they feel is crucial. Working with PAPYRUS has been easy—they’ve been so open and available for questions at a moment's notice."

Our recent Quarter Brief (QB) was a great success, thanks to the efforts of Fooad El-sayed and Oliver McCauley, who led the preparation for this session! Their hard work paid off, resulting in a highly engaging and productive meeting. ��

Kicking Off with Safety: Lessons from Grenfell Tower

We began with a Safety Moment, reflecting on the Grenfell Tower incident—a tragic event that serves as a stark reminder of the importance of safety compliance. The failure to meet safety standards can have devastating consequences, affecting thousands of lives. This topic highlighted the importance of embedding safety into everything we do, not only in our day-to-day operations but in our long-term strategies as well. It was a sobering reminder of why we must stay vigilant and ensure compliance with safety regulations.

Sue McCauley also took the opportunity to emphasize the critical importance of cyber safety and security, stressing how essential it is to remain vigilant in this area to protect both our business and personal environments.

Reflecting on Past Achievements and Challenges

We took a moment to look back at the previous Quarter Briefs, where we shared our achievements and discussed the challenges we’ve faced. It was a valuable opportunity to celebrate our successes and learn from any obstacles along the way. The Senior Leadership Team (SLT) provided important updates on selling objectives, ensuring that we are aligned and working towards our collective goals.

Group Activities and the "Swindon to Toronto" Challenge

One of the standout moments of the QB was the group activities, which helped to foster teamwork and collaboration across the board. A highlight was the announcement of a new fitness challenge dubbed the "Swindon to Toronto" challenge—a fun and motivating way to encourage physical well-being. This challenge is something everyone is looking forward to, and it’s sure to bring out a competitive yet supportive spirit in the team.

Better Every Day: A Focus on Improvement

As part of our overarching theme of "Better Every Day," we discussed how we can continue to improve both professionally and personally. It's not just about optimizing our workflows or achieving our business goals, but also about enhancing our mental and physical health. We are committed to fostering an environment where continuous improvement is at the core, both in and outside the workplace. ��

Supporting Mental Health: A Talk on Papyrus

A special moment during the QB was the introduction of Papyrus - Prevention of Young Suicide, an organization dedicated to supporting young people and preventing suicide. This important topic was suggested by Michael Hinton, and the insightful presentation was delivered by Alex Hills. It was a powerful reminder of the importance of mental health, and we are exploring ways we can support Papyrus through volunteering or fundraising. As a company, we are dedicated to making a positive impact not just in our industry, but in the communities we serve.

The Quarter Brief was a meaningful and inspiring session, leaving us all with renewed motivation and clarity. We are proud of what we’ve accomplished so far, but we know there’s always room for improvement!

How to Work Effectively with Your AsBo and Avoid Pitfalls

Do you have an AsBo?! If so, whatever you do, do not PANIC!!

As I am sure you already know, if a project is deemed ‘Significant’ according to the Common Safety Method for Risk Evaluation and Assessment Regulations (CSM RA), then an independent assessment of the application of the CSM RA risk management process needs to be made by an Independent Assessment Body; commonly referred to as an AsBo.

Now when it comes to working with an AsBo a lot of projects can get quite worked up over it! AsBos can often be seen as being expensive, unhelpful, and all too willing to provide Safety Assessment Reports (SARs) which are NOT supportive of how projects are applying the CSM RA process.

However, often it is the project’s own failings that can lead to multiple reviews being required (time is money after all) and unsupportive SARs being issued.

Early and regular engagement with your AsBo is key, ideally starting as early as the Option Selection stage of your design. This provides the project the opportunity to bring the AsBo on the journey with you and demonstrate how safety has influenced design and hopefully how potential hazards have been eliminated as the project has developed.

Early engagement will also allow the project to present its approach to safety engineering and get the buy in of the AsBo early, helping to make sure that adequate review periods are also built in to your CSM RA submissions schedule (if you don’t have one, get one!). Whilst obviously the independence of the AsBo cannot be compromised, if you listen carefully then the odd hint and tip can be picked up in terms of what they expect to see moving forwards; but only if you are seen to be really putting the effort into demonstrating the safety of your system.

Regular reviews with the AsBo of your key safety assurance documentation (System Definition, System Safety Plan, Hazard Record, HAZID Minutes, Safety Justification Report etc) can help issues be identified early and make sure that everyone is on the same page. If there are issues with respect to the demonstration of safety then make the AsBo aware; it would not be a good thing for an assessor to find out on their own; rightly wondering what else they haven’t been told about.

When its time to submit your Hazard Record and supporting Safety Justification Report to the AsBo prior to commissioning, it is very important to ensure that you are clear on what safety assurance evidence remains outstanding as well as the who, what, when and how this assurance evidence will be made available. Given that pre-commissioning Hazard Records and Safety Justification Reports need to be submitted sometimes up to 15 weeks prior to a commissioning then clearly not all compliance information will be available. This shouldn’t in theory cause too many issues as long as the project can demonstrate it is still in control and you can demonstrate that this evidence is becoming available as promised during the AsBo review period.

Brush Monters STEM Event at Tortworth Primary School

R ecently, Oliver and Oi Ki utilized this opportunity to participate in the Brush Monsters STEM Event at Tortworth Primary School , a fantastic initiative under the Technology for Fun Project .

Oi Ki shared her enthusiastic experience, stating, " I had the opportunity to volunteer for a STEM event, using one of the five volunteering days allowance at HSRS. It reminded me how incredibly rewarding it feels to be part of such an event. Witnessing children come up with creative ideas was truly inspiring, and I’m already looking forward to doing it again soon. "

The Brush Monsters STEM event aimed to ignite a passion for science, technology, engineering, and mathematics among young students. Through hands-on activities and interactive learning, the children were encouraged to explore the wonders of STEM in a fun and engaging way. Oliver and Oi Ki, guided the students, helping them build and program their own 'brush monsters'—small robots made from simple household items.

Common Safety Method for Risk Evaluation and Assessment (CSM-RA) vs CENELEC Standards

Introduction

There are many standards, regulations, and laws that projects must apply when demonstrating safety assurance within the railway industry. As part of this, some of the most common items referred to are the “CSM for risk evaluation and assessment - Commission Implementing Regulation (EU) No 402/2013” (henceforth referred to as the CSM-RA) and CENELEC standards (EN50126/8/9). However, it can be difficult to understand the interfaces between these especially when considered with other applicable standards or regulations.

Before detailing further, we must first clarify some terms. “Safety”, in isolation, is defined as the freedom from unacceptable risk of physical injury. “Functional Safety” on the other hand is more specific - it defines the overall safety which depends on functional and physical systems operating correctly in response to inputs, or failure, in a predictable manner.

CSM-RA and CENELEC (5012X)

CSMs were established in accordance with Article 6 of Directive (EU) 2016/798. Of these CSMs, the Common Safety Method for Risk Evaluation and Assessment Regulation was created to set out the risk assessment process to be applied in case of technical, operational, or organisational changes. It is the starting point for proposing any change within the mainline railway system and its significance must always be considered by the organisation or person making the change (“the proposer”). Note that this legislation does not apply to every railway environment (for example, metros or privately owned infrastructure for freight).

For a change determined as ‘Significant’ CSM-RA defines the requirement for a system definition, system safety plan, hazard identification and management process, risk evaluation, safety requirements management, and demonstration of compliance. In order to provide additional assurance, CSM-RA specifies an independent assessment, by an accredited Assessment Body (AsBo), is required that confirms the correct application of the risk assessment process, the results of the process and the safety demonstration of the system under assessment, that the necessary level of safety can be achieved.

In contrast, the railway specific CENELEC standards are derived from IEC 61508 (Functional Safety of Electrical/Electronic/Programmable Electronic Safety-related Systems). They are technically voluntary; however, national laws often require their application. For example, the Control Command and Signalling National Technical Specification Notice (NTSN, previously TSIs) specifically lists EN 50126, EN 50128, and EN 50129 as mandatory standards. These CENELEC standards are:

EN50126: Specification and Demonstration of Reliability, Availability, Maintainability and Safety (RAMS). This defines the terms of RAMS and a process based on the system V-lifecycle. It includes the activities involved with and responsibilities for the system definition, safety management, specification and management of system requirements, and validation activities. It does not define specific RAMS requirements (e.g. failure rates) and thus can be applied to any generic system.

EN50128: Communication, signalling, and processing systems — Software for railway control and protection systems. The standard provides comprehensive guidelines for rail software development activities through the specification of procedures, technical requirements, and V-lifecycle for the development of programmable electronic systems for usage in railway control and protection applications (safety related software only). This applies to application software, operating systems, support tools, and firmware.

EN50129: Communication, signalling and processing systems – Safety-related electronic systems for signalling. This standard is focused on the evidence to be presented for the acceptance of safety-related signalling systems (i.e. the safety case and associated evidence) and is highly related to the EN 50126. It covers details for the specification, allocation and implementation of safety requirements and safety integrity; the use of Safety Integrity Levels in safety-related systems for railway application; and the overall safety case structure. While originally designed for signalling, it can be applied to other systems as far as is reasonably practicable.

Similarly to CSM-RA, the CENELEC processes require independent assessment, though this is performed by the Independent Safety Assessor (ISA).

How do CENELEC and CSM-RA Interact and When Should we Apply Each?

CSM-RA and CENELEC have commonalities between them; for example, the risk evaluation and management processes for each are almost identical. However, they are aimed at different system application levels.

For example, when developing a new interlocking system for potential use in the railway environment, one should apply the CENELEC standards to demonstrate that the product (generically) meets the RAMS requirements, including the interfaces between each subsystem.

Now let’s consider that a proposer wishes to introduce this interlocking system into the main railway environment. As there is a significant change to the railway, the CSM-RA process must be followed and demonstrated by the proposer. This is used to demonstrate that the risk has been controlled before implementing any changes to the railway (also considers overall operational and maintenance regime changes). To support this, the supplier would also need to demonstrate through EN5012X that the project specific solution for this interlocking system (application data, hardware layout, architecture, etc.) has followed the requirements for RAMS and is safe to integrate into that specific railway environment (scoped to that interlocking system and any interfaces).

At HSRS, we're passionate about making a positive impact in our community and shaping the future for generations to come. That's why we're proud to offer our employees 5 days of volunteering leave each year, empowering them to make a difference in causes they're passionate about.

We're excited to highlight the recent volunteering efforts of two incredible members of our team, Oliver and Josh, who dedicated their time to the Hillesley STEM Workshop! ��

Josh shared his experience: "It was wonderful working with the students at Hillesley. They were not only curious about key underlining principles, but also showed real enthusiasm in designing, testing, and improving their creations. It is important that experienced professionals help to foster these skills and passions, for they are what make for future engineers and future technological developments. Experiences like these also help those professionals to develop their teaching and planning skills which are used throughout any career. I would like to personally thank all the staff at Hillesley VA Primary School for all the support they provided, as well as giving us the opportunity to make a real societal impact."

Oliver also expressed his thoughts: "It was a fabulous day working with the students at Hillesley VA Primary School. All of them showed a tremendous level of enthusiasm and curiosity into learning how the fan boats worked and the process behind building them from their individual components. This is the second time I have done a workshop with these students following the workshop last year creating vibrating brush monsters and I can honestly say that there are few greater feelings of pride in Engineering than knowing I am making a positive difference to these young people’s lives and hopefully inspiring them into a career in STEM. Thanks again to all the staff and students for a wonderful time. Already can’t wait for next year’s workshop."

Through their commitment and enthusiasm, Oliver and Josh have not only contributed to the workshop's success but also inspired countless young minds to explore science, engineering, and technology.

Celebrating National Apprentice Week 2024

This week, from 5 to 11 February, is National Apprenticeship Week 2024. This is an annual event that celebrates the value, benefit and opportunity that apprenticeships can offer to individuals, employers and the wider economy. The theme for this year is “Skills for Life”, which reflects how apprenticeships can help people develop the skills and knowledge required for a rewarding career.

At our company, we are proud to support apprenticeships and the positive impact they have on our business and our industry. We currently have one apprentice working with us, who is learning from our experienced and talented team. We also have several senior staff members who started their career with an apprenticeship and have progressed to become leaders in their fields.

One of them is Jack, our Principal Engineer, who is not only a brilliant Civil Engineer, and an experience Designated Project Engineer and Contractor Engineering Manager.

Jack’s story is an inspiring example of how apprenticeships can provide skills for life, both professionally and personally. He shared his journey in a recent interview:

Why you chose to complete your apprenticeship?

"After doing my GCSE's I went to college to study business accounting but soon realised the classroom, and accountancy, wasn't for me. I therefore looked into getting an engineering apprenticeship to get some hands on experience whilst still earning a qualification. I hadn't planned to go into rail originally but once I found out more about the apprenticeship on offer I was keen to get into the industry."

What qualification did you obtain in the end?

"BSc (Hons) in Civil Engineering and Technician Member of the ICE (EngTech MICE)."

What were the benefits over "conventional route" ?

"My apprenticeship enabled me to get a great mix of experience from drafting and designing to on site surveys and installation as I moved around various teams. I also had a wealth of knowledge on hand to help me with my studies and could implement my learning into reality right away. Plus no debt! "

How did you enjoy your time as an apprentice?

"I was lucky enough to undertake my apprenticeship alongside 2 other apprentices who were a year and 2 years 'above me'. This not only meant I had great support and never felt alone when having to study late, meet deadlines or coming up against difficulties, it also meant that we could arrange site trips, visits to other offices and most importantly socials! I was also fortunate enough to assist a number of experienced engineers throughout my apprenticeship all with varying backgrounds and experience that gave great variety."

How did it help shape your career?

"Being exposed to a variety of different roles and types of works during my early career enabled me to choose a career path that appealed to me most. Being in a working environment from a young age also helped grow my soft skills, from how to conduct yourself in a professional environment to decision making and leadership, all of which I still rely on today."

What a great day we had yesterday at our Quarter 4 Company Brief and Christmas lunch! ��

London's latest station is now open.

Brent Cross West: A New Gateway to London

Brent Cross West is London’s first new mainline station in over 10 years. It opened on Sunday, December 10, 2023, as part of a regeneration project that will create a new town centre with homes, offices, shops, parks, and community facilities.

The station connects Brent Cross Town to central London and beyond via Thameslink services, which can reach St Pancras in 12 minutes. The station also has a green roof, solar panels, and a new overbridge that links Barnet and Brent.

The opening of Brent Cross West was celebrated with a special event, attended by the Mayor of London, local councillors, and the rail industry. The station also had a celebrity guest: broadcaster and Barnet resident Tony Blackburn, who became its inaugural announcer.

HSRS was appointed be MACE, Barnet Council’s Delivery Partner, to provide safety assurance services including authoring the System Definition, System Safety Plan, Safety Justification Reports, and Hazard Record in compliance with Common Safety Method for Risk Assessment and Evaluation (CSM RA) for both GRIP 5 and 6 stages. Our team have also hosted a number of specific Hazard Identification Workshops in order to identify potential risks, which informed the key design choices regarding the station.

Brent Cross West is a welcome addition to London’s transport network and a key milestone in the transformation of the area. It will provide new opportunities for travel, work, and leisure for residents and visitors alike.

MART follows the same structure and logic as CSM RA, but adapts it to the maritime context and data sources.

Road: The European Commission has funded a research project called SAFER-LC (Safer level crossing by integrating and optimising road-rail infrastructure management and design), which aims to improve safety at level crossings by applying CSM RA. The project has developed a web-based tool that guides users through the steps of CSM RA applied to level crossings, from hazard identification to risk acceptance.

Nuclear: The International Atomic Energy Agency (IAEA) has used CSM RA as an inspiration for its safety assessment methodology for nuclear power plants (SAMG), which provides a structured approach for assessing the safety implications of changes or modifications in nuclear power plants. SAMG follows a similar process as CSM RA, but incorporates specific criteria and techniques for nuclear safety analysis.

Conclusion

CSM RA is a versatile and robust framework for risk assessment that can be applied outside the rail industry with some adaptation and customisation. By applying CSM RA to other sectors, different domains can benefit from cross-sectoral learning and collaboration, as well as enhance the consistency and quality of their risk assessment. However, applying CSM RA to other sectors also poses some challenges that require careful consideration and communication with the stakeholders involved in the process.

Successful Commissionings for Brent Cross

The last few weekends have seen a number of successful commissionings as part of the Brent Cross project. This has included a major slew of one of the mainlines that will allow for the construction of the new station island platforms; as well as the commissioning of a new AHB Level Crossing within the Cricklewood Sidings.
